CIRCULAR

It is circulated for the information of the eligible employees of this University that in pursuance of regulation regarding facility for performance of Hajj, applications are invited in prescribed proforma from University employees who qualify the following eligibility criteria for performance of Hajj during the year of 2008 as under:

University employees who have been appointed on Regular Basis BPS – 01 to 16, and they have completed at least 10 years as a University employee by the closing i.e: 09th August 2008.

The prescribed application Forms can be obtained from the office of the Assistant Registrar (Academic) Liaquat University of Medical & Health Sciences, Jamshoro on any working day during office hours or may be downloaded by clicking on the following link:

The last date of application form is fixed as August 09, 2008.

NOTE: NO EXTENSION SHALL BE MADE IN THE LAST DATE FOR SUBMISSION OF APPLICATION PROFORMA UNDER ANY CIRCUMSTANCES.

"DECLARATION OF INCOME, ASSETS AND EXPENSES FOR THE FINANCIAL YEAR ENDING JUNE 30, 2007 (JULY 01, 2006 TO JUNE 30, 2007"

REFERENCE:

With reference to the letters quoted above, all University employees (BPS-01 to BPS-22) working either on Regular basis, re-employed, contractual or working on deputation basis at this University are directed / requested to declare their Income, Assets and Expenses for the Financial year ending on June 30, 2007 (July 01, 2006 to June 30, 2007) on the prescribed proforma, which is available at WWW.ESTABLISHMENT.GOV.PK. Also, the prescribed form can be downloaded by clicking on the following links. The photocopy of the prescribed form is also acceptable.

The Dean / Chairman / Incharge / Director / Head of each Department should fill-in their own declaration form and also direct the persons serving under them to submit their declaration of income, assets and liabilities for the Financial year ending on June 30, 2007 (July 01, 2006 to June 30, 2007) immediately. A certificate to this effect that declaration of Assets Proforma have been obtained from all the employees (Regular / Contractual / Re-Employed / on Deputation) serving under their administrative control may also be forwarded to this office within fortnight positively.
